New Patient Information
At The Smile Loft, your first appointment includes a thorough exam and gathering any information that our dentist must be aware of in order to provide care specifically to your oral health. We understand that all patients are different and require individualized care.
The Smile Loft understands the importance that dental insurance can play in providing care to our patients. We are happy to be in network with many insurance carriers as well as accept out of network benefits from carriers that we are not contracted with. As a courtesy, we will file your dental insurance claim and assist in obtaining maximum benefits from the insurance company. We will provide you with a treatment plan and an estimated “out of pocket” expense for all dental treatment. We ask that you pay this estimated portion at the time of service. Your dental insurance policy is an agreement between you and your insurance company and therefore we can not guarantee any estimated coverage made by your insurance company.
